The applicant must be at least 18 years of age.
The applicant must have an IQ of approximately 69 or below and have significant limitations in adaptive skills in two or more of the following areas: communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work.

The applicant and designated representative will be requested to participate in a personal interview (prior to admission) to review needs, interests, and current goals.
If the individual is accepted, the Developmental Disability Service Coordinator/ or Clinical Director will develop an initial Plan for Support with the assistance of the entire program team that is reviewed and updated quarterly.
The Plan for Supports will have direct input from the individual, caregiver, and Developmental Disability Coordinator/ or Clinical Director.
Our staff will review the needs, interests, and current goals with the individual, their family and any other person centered planning partners.
The Developmental Disability Service Coordinator/ or Clinical Director will identify funding sources for the individual and Plan for Support Services.
The Plan for Supports is to meet the needs and interest of the individual.
